A simple comments form that you can use for your site. To use it you must upload both the mail.php file (included) and the swf file to your server. You must also change the "email@email.com" in the FLA file to your email. Enjoy!

This appears to be a flash file, you cannot see a preview because flash player is no longer supoorted in browsers, but you can still download the file to study it's source code.
You can open/play the swf file with Flash Player exe
